Sports physical forms are located under Menu, Documents, Forms, on this website. Sports physicals will be good for one year - meaning they are good for the 2021-2022 school year.
Cost is $25.00. Make checks out to CMMC (Central Montana Medical Center).
CMMC will host sports physicals at Stanford School on Thursday, May 27th. They will see the first students starting at 8:30 am. Parents or guardians please call CMMC at 538-4110 to schedule a time for their child’s physical. Please make sure students wear comfortable clothes, glasses, and easy to remove shoes.
